Agranulocytosis: Myelogram in Granulopoiesis Reduction

A clinical case of severe neutropenia in a 16-year-old patient: bone marrow aspirate analysis, absence of granulocytes, and reactive plasmacytosis.

🔬Clinical case from a doctor from 🇮🇱Israel - Anna Belen. Suggest a diagnosis. The picture somewhat resembles the previous case of leukopenia, absence of granulocytic lineage. 16-year-old female patient, nasopharyngitis. Peripheral blood shows leukopenia, anemia: WBC 0.94; neutrophils 0.02, 2%; hemoglobin 111; platelets 204. Bone marrow aspirate in the photos. Immunophenotyping: blasts not detected, granulocytes absent. Histology: moderate bone marrow hypoplasia with severe myeloid hypoplasia and myeloid lineage maturation arrest. Increased number of plasma cells. Blasts not detected. Slight erythroid lineage proliferation. Megakaryocytes present, without significant proliferation.
