AML vs MDS. A 32-year-old patient with pancytopenia. In the peripheral blood smear, I count 25% monocytoid cells, some with rejuvenated chromatin. However, I hesitate to call them blasts. I write “Abnormal monocytes” and recommendations for bone marrow aspiration. In the bone marrow, phenotyping shows 12% myeloblasts. Smear count yields 25%. That is, such a diagnostically borderline condition. No signs of MDS were observed on immunophenotyping. Staining for ring sideroblasts is negative. What do you think about the diagnosis? What tests should be decisive for making a decision?
