A 75-year-old patient reports severe fatigue and weakness for about a month. He has not seen a therapist; his medical history only includes inpatient treatment for surgery (hernia repair).
Peripheral blood shows pancytopenia and dysgranulocytopoiesis: neutrophil hyposegmentation is combined with hypersegmentation (macropolicyte); neutrophil hypogranulation is combined with hypergranulation, Harlequin cells. Poikilocytosis: acanthocytes. Platelet anisocytosis: both microplatelets (less than 2 µm) and macroplatelets, giant platelets are present.
Suspected MDS. Hematologist consultation and sternal puncture are recommended.
