A clinical case from this hour’s on-call shift. A 73-year-old patient. Treated for ARVI since January.
🚑 Now admitted to intensive care with bilateral pneumonia and a terminal condition.
🩸 Complete blood count shows the following key changes: Anemia: Red blood cells: 3.83 ×10¹²/L (decreased) Hemoglobin: 115.8 g/L (decreased) Hematocrit: 0.334 L/L (decreased) Elevated white blood cells: White blood cells: 131.22 ×10⁹/L (significantly elevated) Blast cells: 77 % (normally absent) Platelets: 198.9 ×10⁹/L (within normal limits)
Conclusion: In the blood smear preparation, atypical cells morphologically consistent with promonocytes were found. According to ICSH/WHO standards, promonocytes are considered blast equivalents.
High leukocyte levels and the presence of blast cells indicate possible leukemia. ⚠ Urgent hematologist consultation required!
