Neutrophil Hypersegmentation. Hypersegmentation of segmented neutrophils means the nucleus has more than five lobes connected by a thin chromatin filament. Hypersegmentation is defined when any neutrophil has more than 6 lobes or more than 3% of neutrophils have more than 5 lobes. Neutrophils showing grape-like nuclei (botryoid neutrophil) are observed in cases of severe burns, hyperthermia, and cocaine abuse. Some other causes of hypersegmentation include: megaloblastic (pernicious) anemia; vitamin B12 and folic acid deficiency; macrocytic anemia; myelodysplastic syndrome; leukemias; purulent-inflammatory infections; radiation sickness; chemotherapy.
