Patient, 50 years old. Undergoing examination with a diagnosis of “fever of unknown origin”. In CBC, leukocytosis, WBC 10.62×10 9/L, with a shift towards neutrophilic granulocytes. In the myelogram, photos 1-10, the granulocytic lineage is irritated, without maturation disturbance. The number of ECs is slightly increased, erythropoiesis is normoblastic. The functional activity of the megakaryocytic lineage is preserved. In photo 6 - a macrophage with vacuolated cytoplasm, phagocytosed contents (neutrophilic granulocytes, erythrocaryocytes). In photo 10 - a megakaryocyte with excessive platelet shedding and emperipolesis (engulfment of other hematopoietic cells - presence of two granulocytes).
