Lymphocytes with Cytoplasmic Projections in Blood Smear

A clinical case of pronounced hyperleukocytosis in an 85-year-old female patient: morphology of atypical lymphoid cells with cytoplasmic projections in a blood smear.

Anonymous question from a subscriber. Blood smear, female, born 1938, diagnosis unknown, primary visit, home visit. Romanowsky-Giemsa stain. 1000x magnification. What are these lymphoid elements with “whip-like tails” of cytoplasm? I haven’t encountered them in literature. How should they be properly described, if at all… It’s clear that the nuclei are convoluted with a cerebriform structure.
