MDS with Ring Sideroblasts: Bone Marrow Cytology

A clinical case of MDS with ring sideroblasts: pancytopenia in peripheral blood, erythroid hyperplasia, and Perl's stain of bone marrow aspirate.

I suggest considering the diagnosis. Patient is 68 years old. CBC shows pancytopenia: WBC 2.1; PLT 43; Hb 57, RBC 1.91; peripheral blood also shows erythroblasts 10/100 WBC, reticulocytes 1.8%, macrocytes, ovalocytes, neutrophil hypersegmentation. Bone marrow aspirate in photos 1, 2, 3. Its immunophenotyping shows: 74% erythroblasts, 5% myeloblasts, 3% granulocytes, 3% monocytes, 11% lymphocytes. Cytochemistry for sideroblasts in photo 4 (the only cytochemistry we perform, and very rarely, at the request of hematologists).
