Neuroendocrine Carcinoma Metastasis to Bone Marrow

A clinical case of large cell neuroendocrine carcinoma metastasis to the bone marrow: cytology of trephine biopsy imprints in pancytopenia.

⚠ Large cell neuroendocrine carcinoma metastasis to the bone marrow. 👉Following up on the post about bone marrow metastases, Anna Belen (a doctor from Israel🇮🇱) kindly sent us another case. ✅A 79-year-old male with pancytopenia in peripheral blood (analyzer photo below) and LDH 14930 U/L (reference interval: 230-480). ✅Mediastinal lymphadenopathy was also found during examination. ✅Preliminary diagnosis: lymphoproliferative disease. ✅Bone marrow aspiration failed; imprints were made from the trephine biopsy (photos below). ✅Histological conclusion from bone marrow biopsy: large cell neuroendocrine carcinoma. ✅Anna Belen advises in such cases, when doctors fail to obtain adequate material and when a hypocellular bone marrow is observed in the smear, to look for cell clusters at low magnification, especially along the edges of the smear, to detect malignant neoplasm cells.
