Clinical case from a subscriber.
A patient from the pulmonology department. Admitted with complaints of fever up to 39 degrees Celsius, shortness of breath, and chest pain during deep breathing. Ill for 3 weeks. At home, took Sumamed without effect. From the medical history: smoked for 50 years, quit 1 month ago. In the summer, experienced weakness and pain in the lumbar spine. Hemoglobin 105 g/L. Pain appeared in the thoracic spine and ribs. Admitted by ambulance with suspected pneumonia. MSCT of the chest organs showed a picture of left-sided bronchopneumonia S4. Multifocal lesions of the thoracic vertebrae and ribs with signs of destruction. Consulted by a hematologist. The myelogram showed an increase in the number of plasma cells up to 18%, with binucleated cells present. Erythrocytes showed aggregation in the form of “rouleaux”.
