Patient, 39 years old, fell acutely ill two weeks ago, visited a dentist for caries and periodontitis, after which she began to notice bruises on her body, but was not examined. On April 26, 2015, a complete blood count was performed, which showed leukocytosis of 13x10*9/L, hemoglobin 114g/L. Admitted to the hematology department as an emergency. Upon admission, the leukocyte differential count showed 27% blasts. Material: bone marrow. Blasts 87%, blasts of medium size, with oval or round nuclei, basophilic vacuolated cytoplasm. Other hematopoietic lineages are suppressed. Morphologically, the blasts resemble the L3 variant of acute lymphoblastic leukemia. Cytochemical reactions for myeloperoxidase, lipids - negative, PAS reaction granular in 2% of blasts.
