AML Morphology with t(8;21)(q22;q22.1) RUNX1-RUNX1T1 Translocation

Morphological features of blasts and granulocytes in acute myeloid leukemia with t(8;21): Auer rods, pseudo-Pelger-Huët anomaly, and azurophilic granulation.

Acute myeloid leukemia chromosomal rearrangement: translocation t(8;21)(q22;q22.1) RUNX1-RUNX1T1.

Blastic cells are predominantly of macro-generation. Blast nuclei are pleomorphic: round, reniform, lobulated. Nuclear chromatin is coarsely dispersed, with 2-3 nucleoli visible. The cytoplasm is abundant, predominantly basophilic, sometimes with a round perinuclear clearing (Golgi zone); cytoplasmic vacuolization may occur. Most blasts have azurophilic granulation in their cytoplasm; Auer rods, pseudo-Chédiak-Higashi-like granules, and large orange-pink globules (sometimes compared to salmon color) are observed. Furthermore, in this AML variant, mature and maturing cells of the neutrophilic lineage exhibit atypical nuclear segmentation, including pseudo-Pelger-Huët anomaly. Auer rods and orange globules may also be found in these neutrophils.
