A patient with pancytopenia was admitted to the hematology department. Bone marrow aspirate showed total blast infiltration. Blasts had hyperchromatic nuclei, poorly discernible, indistinct nucleoli, and abundant azurophilic granularity in the cytoplasm. Blast cells with bundles of Auer rods were observed. Myeloperoxidase was strongly positive in 100% of blast cells. Acute leukemia of myeloid origin was suspected (promyelocytic?). Immunophenotyping and molecular genetic studies were recommended to confirm the diagnosis.
