Patient, 61 years old. CBC shows anemia, thrombocytopenia, monocytosis (monocytes, promonocytes), photos 1,2,3.
Bone marrow aspirate shows blastosis. Most blast cells have monocytoid nuclear shapes, indistinct nucleoli; some blasts have round, oval nuclei, and distinct nucleoli.
Acute leukemia of myeloid origin (myelomonoblastic variant) can be presumed. Cytochemistry and immunophenotyping are recommended.
