The patient was admitted for examination with complaints of weakness, fatigue, and fever up to 38°C. Antibiotics and antivirals had no effect. A complete blood count revealed leukocytosis up to 31x209/L, platelets 121x109/L. In the peripheral blood (result sheet in the last photo), blasts are 73%, some containing Auer rods. Two types of blasts are noteworthy: one with round nuclei, and the second with nuclear polymorphism, indentations, bridges, and bizarrely shaped nuclei. The myelogram shows 80% blasts. Cytochemical reactions – myeloperoxidase, lipids – are positive in 40% of cells (photos 8, 9), diffuse glycogen. According to immunophenotyping results, blasts are 75%, confirming the myelomonocytic variant of acute leukemia.
