Primary Myelofibrosis: Blood and Bone Marrow Picture

Primary myelofibrosis: anemia, dacryocytes, and left shift in blood; erythroblastic islands and atypical megakaryocytes in bone marrow aspirate.

In a patient with primary myelofibrosis, the peripheral blood shows anemia, thrombocytopenia, a left shift of the leukocyte formula to myelocytes (photo 1), and normoblasts (photo 2). Anisocytosis and poikilocytosis with teardrop cells are noted. In the bone marrow aspirate (photos 3-8), erythroblastic islands (a macrophage surrounded by erythrokaryocytes) and atypical megakaryocytes are found. Photo 6 shows a megakaryoblast?
