Plasma Cells in Blood Smear in Multiple Myeloma

A clinical case of detecting 6% plasma cells in a complete blood count in myeloma before bone marrow aspiration and a sharp increase in free lambda chains.

Very rarely in myeloma do plasma cells enter the peripheral blood. But in this case, the diagnosis began with the detection of plasma cells in the smear. There were 6% of them. We differentiate them from lymphocytes and erythroblasts. WBC 11.6; Hb 113; PLT 212. Further, light chains were detected (see photo) Bone marrow aspiration had not yet been performed.
