Systemic Mastocytosis: Clinical Case and Morphology

A clinical case of systemic mastocytosis: splenomegaly, urticaria pigmentosa, blood tryptase level >200 µg/L, and atypical mast cells in the bone marrow.

⚠Systemic mastocytosis. Clinical case. ✅A 42-year-old male with significant splenomegaly, hepatomegaly, and urticaria pigmentosa-like skin rash. 🔬Peripheral blood showed mild anemia: hemoglobin 113 g/L and leukopenia 3.11×10^9/L. ✅Bone marrow aspiration for IFT failed. Touch imprints from the trepanobiopsy were made: we see clusters of atypical mast cells. ❗Blood tryptase >200 µg/L (N = 0-11.4) – a marker of mast cell activation, a marker of mastocytosis. ✅Histology showed bone marrow infiltration by mast cells positive for CD117, CD25, CD2.
