MDS to AML Transformation: Bone Marrow Cell Morphology

Bone marrow morphology in MDS transforming to AML: 23% blasts, trilineage cytopenia, dysplasia, and cytochemical studies with MPO and PAS reaction.

Bone marrow smears from a 64-year-old patient diagnosed with MDS, with trilineage cytopenia in the periphery, 23% blasts, predominantly myeloid, some promyelocytic type cells, megaloblasts, inactive megakaryocytes. Transformation to AML is likely. Cytochemistry: MPO positive, PAS in diffuse form. Immunophenotyping recommended to confirm the diagnosis.
