Granulocyte Vacuolization and Emperipolesis in Bone Marrow

A clinical case of leukocytosis with neutrophil vacuolization, thrombocytopenia, and emperipolesis in bone marrow in a patient with acute leukemia post-BMT.

In the patient’s complete blood count, there is leukocytosis with a left shift and thrombocytopenia. Most neutrophilic granulocytes show cytoplasmic vacuolization, which is apparently related to the severity of the pathological process. The patient’s history includes acute leukemia and bone marrow transplantation. In the bone marrow aspirate, individual granulocytes are vacuolated, similar to those in peripheral blood. Macrophages (photo 8) and megakaryocytes (photo 9 - emperipolesis) with phagocytosed cells are observed. The patient’s examination continues (bacterial cultures, etc.), apparently to rule out infections and clarify the cause of hyperleukocytosis with a left shift and the severity of the condition.
