Granular Casts in Urine Sediment: Microscopy and Genesis

Morphology and clinical significance of granular casts in urine sediment. Causes of formation, differential diagnosis in renal pathology.

Microscopy of urine sediment. Granular casts. Granular casts are opaque, with a fine or coarse granular structure, yellowish, yellow, or almost colorless. Coarse granular casts form from the breakdown of renal epithelial cells, while fine granular casts form from the breakdown of neutrophils or protein coagulation due to changes in physicochemical conditions in the tubules. They are found in glomerulonephritis, pyelonephritis, tuberculosis, kidney cancer, scarlet fever, osteomyelitis, and other conditions.
