Severe Hypochromic Microcytic Anemia in Blood Smear

Patient: 70-year-old female. Peripheral blood picture:

Severe anemia: hemoglobin 44 g/L. Erythrocytes are hypochromic, microcytic, with marked anisocytosis, aniso-chromia, and poikilocytosis.
The smear predominantly shows anulocytes (ring-shaped erythrocytes); echinocytes, dacryocytes, and leptocytes (target cells) are also noted.
Normoblasts: Found in a quantity of 2 per 100 leukocytes.
Thrombocytosis.

🔬Megakaryocyte in 🩸 peripheral blood. ✅A 46-year-old woman❗This is the third case of primary myelofibrosis (PMF) this week from our subscribers. ‼And the second case in the history of the community where we saw a megakaryocyte in the blood. 🧬A mutation in the V617F JAK2 gene was found - 34.7%. 👉Final diagnosis: PMF in fibrotic stage MF III 📸In photos 1,2,3 - peripheral blood microscopy; 4 - complete blood count with reticulocytes.
