LEFT:
The left column illustrates abnormal monocytes that do not meet the criteria for promonocytes (blast cell equivalents) or monoblasts.
RIGHT:
Monoblasts are shown in the right column.
MIDDLE:
The middle column shows various promonocytes with nuclear fine chromatin, similar to monoblasts, but with nuclear folds.

CMML Transformation to Acute Monoblastic Leukemia: A Case

Transformation of chronic myelomonocytic leukemia to acute monoblastic leukemia. 🔬A clinical case from a doctor in 🇮🇱Israel - Anna Belen↗.
A 68-year-old patient. In January 2023, CMML was diagnosed. Currently, leukocytosis is 229×10^9/L. The leukocyte count jumped from 18,000 to 56,000 within 2 days, and within the last 6 hours, from 56,000 to 229,000. The patient passed away on the same day.
In the peripheral blood smear, there is total blastosis.
Blast phenotype: CD64,33,36,38 positive; CD34,117,56,13,HLA-DR,3,19 negative.
