✅ Microscopy of the urine sediment revealed hematuria, predominantly dysmorphic erythrocytes; leukocytes - up to 20 per HPF; renal epithelium with fatty degeneration. An abundance of various casts: red blood cell, granular, epithelial, hyaline, and waxy.

Granular Casts in Urine Sediment: Microscopy and Genesis

Microscopy of urine sediment.
Granular casts.
Granular casts are opaque, with a fine or coarse granular structure, yellowish, yellow, or almost colorless. Coarse granular casts form from the breakdown of renal epithelial cells, while fine granular casts form from the breakdown of neutrophils or protein coagulation due to changes in physicochemical conditions in the tubules. They are found in glomerulonephritis, pyelonephritis, tuberculosis, kidney cancer, scarlet fever, osteomyelitis, and other conditions.

Urinary Casts in Renal Failure: A Case of Massive Cylindruria

Cylindruria in chronic renal failure.
